PVSEC Building is just West of I-279 (Exit 8) on Camp Horne Rd.
$25 per dog (cash or check only, no CREDIT CARDS)
- Please bring AKC registration papers/numbers.
- Arrive 10 minutes before appointment time for cardiology only appointments, come 20 minutes early if also doing ophthalmology CERF to allow for pupil dilation.
- Dog are recommended, but not required to have the following to receive a COMPLETE registered (submitted) OFA Certificate and Number (recommendation made by OFA):
- Microchip, or
- Tattoo
Note: Echo/Doppler Clinic planned for sometime in March -- call for an update.

NON-REFUNDABLE PRE-PAYMENT (check or money order) at time of appointment scheduling.
$36 for the first dog; $26 for each additional dog per owner.
- Please bring AKC registration papers/numbers.
- Arrive 20 minutes before appointment time to
allow for pupil dilation.
- Dogs will need to have one of the following for a complete registered (submitted) CERF Certificate and Number (rule made by CERF at Purdue University):
- Microchip,
- Tattoo, or
- AKC DNA Profile
